Lancaster: We seek a skilled Nurse Practitioner to offer PALLIATIVE CARE and HOSPICE services on behalf of a respected critical access hospital in picturesque northern New Hampshire, near the White Mountain National Forest. Become part of a clinical team offering surgical, ER and ICU services along with a variety of outpatient care. You will join an active hospice/palliative care program based in Littleton, NH. They support patients from 4 hospitals, offering home care, palliative care, and hospice services across two counties. Clinical teams are on call 24/7 with the mission of “providing quality home health and hospice care, utilizing a holistic approach, while working in collaboration with all community resources, to meet the comprehensive needs of the clients and their families, in a cost effective manner.”

 

This position comes with a convenient 4 day/week schedule (32 weekday hours is full time) with half of your time booked in Lancaster, and the other half split between Berlin and Colebrook (with paid mileage). Your actual clinical hours will vary from week to week based on referrals and a variable case load. You will see patients in the hospital setting, in the home, in clinic, and via telehealth appointments – supported by a team that includes an RN and an MA. You will share phone-only call to support hospice patients one week/month.
